Lipoedema stages: Which stage affects you?

Many women suffer from stubborn fat deposits on their legs, arms or hips that just won't go away. In this case, it could be lipoedema. Anyone who receives a diagnosis should therefore not be shocked, but should be pleased that effective therapies can now finally be tackled. Before treatment, however, it is important to first determine the stage of lipoedema.

Stage 1: Painful symmetrical fat distribution disorder

In stage 1 lipoedema, many women confuse their condition with being overweight. This is why they do not visit the doctor, but instead try exercise and a change in diet. Some also suspect cellulite as the cause.

The symptoms of stage 1 lipoedema:
  • Smooth skin that tends to develop orange peel skin when pushed together
  • Thickening of the lower skin layers
  • Symmetrical densification of fatty tissue that can be felt and hurts
  • No to slight swelling

Fat accumulation in the abdominal area is not usually attributable to lipoedema, but it is on the arms and legs. In addition, the fat distribution disorder causes pain at every stage of lipoedema. Even in lipoedema stage 1, there can be impairments in everyday life.

Although lipoedema cannot be cured even in stage 1, those who consult a doctor early on can start suitable treatment as soon as possible. Various measures such as lymphatic drainage, compression or liposuction are used, which our interdisciplinary team specialises in. You should also get enough exercise: sport helps to prevent lipoedema from progressing to the next stage.

The symptoms of stage 2 lipoedema:
  • Wavy, uneven skin surface including dents
  • Thickened areas of skin with nodules (ranging in size from walnut to apple-sized)
  • Disproportion between torso, legs and arms increases
  • Development of riding breeches or Turkish trousers

Stage 2 lipoedema is already more advanced, as the initially slight thickening continues to spread. The body contours become deformed and the fat deposits become larger. It is also typical of stage 2 lipoedema that pain, swelling and sensitivity to pressure increase more and more.

Typical signs of stage 3 lipoedema:
  • Strong increase on the legs
  • Hardening of the tissue
  • Dewlap formation: coarse fat flaps with overhanging tissue
  • X-legs and gait disturbance

Lipoedema stage 2 and lipoedema stage 3 merge seamlessly into one another. There can be years in between. However, hormonal fluctuations can also cause lipoedema to reach stage 3 more quickly. This is why lipoedema can worsen during pregnancy, puberty or the menopause.

Types of lipoedema: In which areas of the body do the fat deposits occur?

Lipoedema can be divided into stages, but also into types that differ depending on the body region. Lipoedema usually starts on the hips and then spreads down the legs and can even reach the lower legs and ankles.

How does stage-appropriate treatment work?

We initially endeavour to take conservative measures, which include compression stockings and manual lymphatic drainage. This alleviates the pain, but does not reduce the fat cells. These can only be removed surgically by liposuction. Liposuction is recommended at an early stage of lipoedema as it improves the patient's quality of life.
